More Pre-fill URL options

I want to create a pre-filled URL to a form with the same structure, but change rich texts, select input options, header & icon images to that specific URL. We use too much forms here, and maintanance on the already created ones is starting to become a bit of a hussle, so i want ways to personalize forms based on URL.
