Form email integration not working

Hello,

I’m not getting email notifications from my form submissions for some reason. Attached is how I set up the integration. I just want certain info to show up in the email sender and subject fields so my email filters will assign results to different folders depending on their answers. But I’m not getting email notifications at all. Any help is appreciated!
